WHAT WE NEED FROM FAMILY
In order to file a dependency claim petition for the surviving family members we need to show that they were wholly or partially dependent on the decedent. For example a spouse, minor children, special needs child, mother or father. Documentation to support those relationships such as:

2. Death certificate
3. Birth certificates (for children and decedent)
